Cryolipolysis of Coolsculpting
The intense cold applied to the skin induces an inflammatory reaction that causes a destruction of the fat cells.
Zeltiq uses the Pelletier effect, which allows the handpiece to act as a heat absorber and thus cool the treated area.
The skin is first protected from cold-induced burns by a film of gel. The integrity of this film is very important to avoid a direct contact of the skin with the handpiece. It is also very important that the operator correctly positions the device because the coolsculping is equipped with a handpiece that sucks the fatty zone into a cavity, to better cool the treated area.
In case of misplacement, sometimes within a few centimeters, the treatment will be less effective or there will be more side effects.
There are different sizes of handpieces adapted to the treated area by cryolipolysis. Nevertheless for large areas such as the abdomen it will move the handpiece 2 to 4 times.
For the outer thighs the handpiece is flat therefore less effective for cooling.
UltraShape V3 Focused Ultrasound
Ultrasounds are inaudible sound waves that propagate in biological tissues.
Ultrasound used in ultrasound passes through tissues without damaging them because of their very low power.
Focused high intensity ultrasound allows destruction of a predetermined biological tissue. This technology is used in medicine to treat tumors or to destroy stones that often allow an alternative to surgery.
Focused ultrasound is of high intensity and low frequency just like those used. The effect obtained is only mechanical which makes the procedure painless.
A cavitation effect obtained by ultrasound convergence appears at the level of the target tissue and permanently destroys a volume of a few cubic millimeters like a rice grain.
To treat the entire predefined fat zone the number of shots can go up to 900.
To precisely juxtapose all shots, the UltraShape V3 has a computer coupled to an integrated imaging system that displays the screen shots to be performed. Each point is the object of only one shooting per session, which allows to get a homogeneous result without risk of hollow or bumps as in a liposuction.
In order to make this machine safer, more precise and more effective three treatment depths (superficial, medium and deep) are available and can be combined in the same session depending on the thickness of fat to be removed.
On the other hand the UltraShape V3 is also equipped with an additional radio frequency handpiece from the Velashape III. After 10 to 15 minutes before the session of UltraShape it optimizes the penetration of ultrasound.
Duration of the treatment

The duration of the treatment is relatively long since the Coolsculpting can only treat one zone at a time and each zone requires the application of the handpiece for one hour. This implies one hour treatment duration for one zone and 4 hours for the 4 zones of an abdomen.

The duration of treatment per session is an average of one hour for an abdomen and one hour thirty for two areas (love handles).

Effects and consequences of treatment
Coolsculpting
Cryolipolysis causes an inflammatory phenomenon It begins 3 days after the session, with a maximum between the 14th and the 30th day. This inflammation persists for up to 90 days. It is during this phase of inflammation that the fat cells die and are digested by the body.
In theory the surrounding tissues are not affected; nevertheless, reversible side effects are described:
- Skin rash (redness)
- hematoma
- pain
- Sensation of local discomfort, tingling
- Rigidity of the treated area
- Increased local sensitivity that may require taking analgesics
- Possibility of a decrease in cutaneous sensitivity that may persist for 1 to 8 weeks resulting in damage to the superficial nerves reversibly damaged by cold.
- Panniculitis: deep painful nodule with erythema and surrounding edema requiring cortisone or anti-inflammatory drugs. This side effect can lead to social exclusion. This effect is even more common when you treat areas such as arms or back or knees.

The elimination of fat is done through the natural processes of elimination of the body.
The destroyed fat cells give on the one hand cell debris and on the other hand the fat composed of triglycerides.
Cell debris is digested by cells called macrophages
